How to write a simple workplace risk assessment

Start with the task, not the form
Describe what people actually do. Walk the job if you can. List hazards you can see, hear or foresee—not only the ones that fit a template.
Who might be harmed
Include employees, contractors, visitors and anyone else affected. Note anyone who may be more vulnerable.
Controls that stick
Prefer controls people can follow under real conditions. Name who will do what, and by when, for actions that are not already in place.
Review triggers
Set a review date and also review after incidents, near misses, new equipment or process changes.
